UVA Health Mobile Care is designed to support any patient who experiences difficulties in accessing traditional healthcare services. To directly support efforts to meet UVA Health's strategic goal to cultivate healthy communities, Mobile Care provides primary care medical services using a data-driven approach to identify patient populations with unmet health needs.

At the heart of UVA Health Mobile Care is our mobile care van and a dedicated care team composed of providers from the UVA School of Nursing, RNs, paramedics, and providers from Population Health; and team members from patient access, financial assistance, interpreter services, and UVA Health Pharmacy. Together, they offer a wide range of services, including general health check-ups, chronic disease management, lab tests, and care for individuals at all stages of life, among other services.

Mobile Care can help patients…

  • If they don't have a regular care provider by providing the initial round of care, and then helping patients establish care at a clinic.
  • If they would like to be seen for a non-emergency situation and cannot wait for an appointment with their regular care provider.
  • By serving as their primary care provider if they do not wish to establish care in a clinic.
